Adventure Pulp Magazine Cover, May 1959
· May 1959

This document is not a pulp magazine but rather the U.S. Air Force's Project 10073 UFO reporting forms from the 1950s. The material consists of standardized questionnaires and record cards used to collect witness accounts of unidentified aerial phenomena. The forms include Project 10073 Record Cards with checkboxes for classifying sightings as balloons, aircraft, or astronomical phenomena; Technical Information Sheets requesting detailed witness observations; and an ATIC Form 329 requesting personal information and narrative descriptions. One legible entry describes a bright light larger than a star observed over Wayne Avenue. The forms systematically gather data on observation circumstances, object appearance, motion characteristics, and witness certainty levels, designed for Air Force analysis and archival purposes rather than entertainment content.
