This document comprises official UFO sighting report forms from two separate investigations conducted by Uruguay's Centro de Investigación de Objetos Voladores Inidentificados (C.I.O.V.I.).
The first record, dated October 31, 1958, from Ventura Mitrelles de Bourdin, describes a luminous, transparent object observed in flight moving eastward at uniform speed with white-blue coloration. The witness notes insufficient data to determine material composition. The form systematically documents observational details: trajectory, duration, altitude, weather conditions, sound, and any physical effects on the observer or surroundings.
The second sighting report, dated October 4, 1958, also from Uruguay, documents an object partially metallic in appearance, observed through binoculars moving on a horizontal trajectory from north to south. The witness describes pale white coloration and normal flight characteristics. The form similarly captures comprehensive data regarding the object's movement, appearance, environmental conditions, and the observer's subjective impressions.
Both forms represent standardized investigative documentation seeking objective physical evidence while acknowledging observational limitations.
