This document consists of two distinct parts. The first is a U.S. Air Force Technical Information Sheet—an official UFO sighting questionnaire containing numbered sections addressing observation details: date, time, location, duration of sighting, sky conditions, object characteristics, motion patterns, size estimation, witness information, and environmental conditions. Questions employ multiple-choice answers and request sketches of observed phenomena.
The second portion is a dated letter (October 14, 1954, though signed from Boulder, Colorado in what appears to be a later period) addressed to Mr. Hynek at Northwestern University's Dearborn Observatory, describing a detailed UFO sighting near Longmont, Colorado in July 1957. The witness, while hay-raking on his farm, observed a bright, shiny craft resembling "blue glacier ice" with a honeycomb-textured top containing mercury-like liquid, a scalloped midsection band, and kidney-shaped designs on its smooth silver bottom. The craft initially descended abruptly, hovered with a rocking motion for ten minutes, then ascended and accelerated southwestward at tremendous speed. The witness estimated altitude around 400 feet and diameter 150-200 feet. A second sighting occurred two weeks later. The witness claims no natural explanation accounts for the observation.
